Our practice provides third party health care providers with a copy of your medical history to ensure continuity of care, such as when we refer you to a specialist. We will respond to requests to transfer a copy of your medical records to another practice. The request may be submitted to our practice by the new practice on behalf of you, the patient. There is a charge of $20 to transfer notes to another practice to cover administration and postage costs.
